The facility, located at 13 Żegańska Street, was constructed in 14 months. The free-standing building has three storeys, and is located close to public institutions of the district.

The outpatient clinic includes:
- on the ground floor there is a Primary Care Physician Clinic for Children - three doctors' offices, and a registration hall.
- on the first floor there is a Primary Care Physician Clinic for Adults - three doctors' offices, and a treatment room,
- on the second floor there are two specialist doctors' offices, a gynaecologist's office, an obstetrician's office, and two dentists’ offices.
The building is adapted to the needs of disabled persons.

The new outpatient clinic is well connected to transport networks (public buses and railways) and located near public institutions such as the Wawer District Office, a nursery school, a primary school, the Wawer District Culture Centre, and a public car park.

Basic healthcare services were moved there from the building at 2C Żegańska, which failed to comply with the technical and sanitary requirements set for outpatient healthcare providers.

The total value of the construction works amounted to PLN 3.95 million, including funding from the City of Warsaw Budget in the amount of PLN 1.5 million. Deputy Mayor of Warsaw Michał Olszewski participated in the clinic’s official opening ceremony on 27 September 2019.
